🏢 Providers — the facilities and organizations you visit
These are the places — SNFs, ALFs, home health agencies, hospices, and more. Track your visit history, referral counts, and key contacts at each location. Use the sub-tabs above to browse by type, or search everything here. The people who refer to you live in the Referral Sources tab.
Show:
Primary targets: Maplewood, Brookhaven, Summit View, Cedarwood, Valley Ridge. Rehab hospitals: Riverside Rehabilitation Center and Summit Health Rehabilitation Hospital. Weekly follow-up with case managers and admissions staff.
🏡 Focus on CCRCs first — Heritage Pointe Senior Community, Ridgemont Village, The Oaks at Clearwater — these campuses include SNF and have the highest hospice conversion potential. Schedule monthly in-service events in these buildings.
🏢 All 32 ALFs in Greenville County. Memory care facilities are your highest-priority hospice referral sources — toggle the filter below to focus on them first.
🏠 Home care companies are strong palliative referral sources. Build relationships with care coordinators and schedulers — they see patients daily and recognize decline early.
Home Care Companies 0 sources
🩺 Home health agencies share patients who are approaching end of life. Nurses and therapists on these teams are often the first to recognize when a patient needs hospice conversation.
Home Health Agencies 0 sources
💙 Other hospice organizations, palliative care programs, and senior placement agencies. Track competitors and partners — relationships here can generate cross-referrals and co-marketing opportunities.
Hospice & Palliative Orgs 0 sources
Physicians, hospitals, senior placement agencies, personal sphere of influence, and any other referral source that doesn't fit another category. Catch-all for your full network.

Morning Social Check
Work down this list each morning. Click Facebook or LinkedIn to open the real page. Stamp ✓ when done — the dashboard tracks how many days since you last checked each facility.
💡 What to look for: Census signals (beds available, same-day admissions) = call today. Hiring signals (CNAs, LPNs) = census pressure, great time to visit. Event posts = attend or offer to co-sponsor. New administrator announcements = re-introduce yourself immediately. Log anything meaningful as a Visit Note on the provider tile.
📍 Weekly Route Planner
Drag facility cards onto days to build your week. Click 📍 Open in Maps to launch turn-by-turn directions. The system scores every facility by relationship tier + days since last visit to suggest your highest-value stops first.

👤 Referral Sources — the people who send you referrals
These are the individuals — discharge planners, DONs, physicians, care coordinators — who personally refer patients to you. Unlike the Providers tab (which tracks facilities), this tab tracks the relationship with the person: how recently you connected, how many referrals they've given you this month, and what's next. Referral Sources are auto-created when you log a referral from a provider tile. You can also add them manually with + Add new source.

💡 Daily habit: Every morning, check this tab. Call or message any source showing "Overdue" before you start field visits. A 2-minute update call ("Just wanted to let you know your patient from last week was admitted and is doing well…") builds the relationship more than any lunch meeting.
Referral pipeline
Every referral received — log it on a facility tile, track it here, confirm the close to post it to Goals.
